Aotearoa New Zealand

Every care conversation, clearly understood.

One Continuum helps older New Zealanders and their whānau follow their care from one conversation to the next, in words that make sense to them.

Kōrero Clear

We translate. We don’t summarise.

Kōrero Clear keeps the full meaning of what was said and puts it in a form the person can genuinely understand.

What was said in the appointment
“We’ll titrate your antihypertensive down and repeat your eGFR in six weeks before we consider re-escalating.”
What it means for you

Your doctor is lowering the dose of your blood pressure medicine.

In about six weeks you’ll have a blood test to check how well your kidneys are working. After that, your doctor will decide whether to change the dose again.

Illustrative example. Our prototype uses synthetic content only.

Care breaks down in the gaps between people.

An older person living at home may see a GP, a specialist, a pharmacist and a community nurse in the same month, while family try to keep track from a distance. Each conversation happens in a different place, and much of what is said is never fully understood.

One Continuum is building clearer pathways and structured oversight, using digital tools with care, so that the person at the centre and the people who support them can follow the whole journey.

Health and aged care

Kōrero Clear

An accessibility companion for health conversations. It translates what was said into accessible language and, with the person’s consent, shares it with the whānau who support them.

How we protect information

  • The person controls the record

They decide who sees what, and can change their mind.

  • Keys held in New Zealand

Each person’s information has its own encryption key, held in Aotearoa.

  • Synthetic data only, for now

No real patient information is used until production safeguards are in place.

Kōrero mai. Let’s talk.

We’re talking with providers, community organisations, professionals and families who want to help shape this work.
